They need the following:
"Kindly send us a scanned copy of your card showing the membership statement, member's name, membership number, membership tier and the tier validity/ expiry date visible enough validity beyond 6 months from the date of requesting status match and flights taken and earned miles on."
Sent them a screenshot of my Miles & Smiles Elite card from the TK app and an overview of my flights of the past 12 months and got a status match to Gold within 1 day. 1 year validity.
