• An attached copy of your other airline's 2020 year end summary listing your name, membership number and elite qualifying miles, segments and dollars (.pdf, .tif, .jpeg, .bmp formats, not to exceed 1 MB)
• An attached copy of your other airline's current 2021 membership card - expiration date of January 2022 (.pdf, .tif, .jpeg, .bmp formats, not to exceed 1 MB)
To expedite your processing time, please be sure to include all of the requested information and materials. Once we receive your documentation and review your eligibility, we will send you an email (typically within 10 business days).

You must earn 12,000 elite qualifying miles (EQMs) and $1,500 elite qualifying dollars (EQDs) OR
14 elite qualifying segments (EQSs) and $1,500 EQDs during the 90-day period.
