I sent a copy of my current United digital membership card and a copy of my passport to [email protected] and got a response the next day confirming a match to Volare "Premium". No membership card (or even a number) has been issued yet by ITA, but you can use the email they send you as proof of status in the interim if flying with ITA. Status will be valid until 31 December 2022.
