Put in a request through the smiles and miles feedback form and attached scan of passport, screenshot of activity from flying blue and screenshot of digital card.
Heard back within 5 working days requesting me to download the digital card and send a scan or a photo. I thought this was redundant, but followed their request anyway.
I was granted Elite status a few working days after.
