Send email to [email protected] [email protected] [email protected] that includes your passport, itinerary, proof of status with UA and they’ll match. Took me about a week calling back and forth because their system was down, but once their systems were up and running, they matched it no problem.

Went to National, created a regular account first. Then after 24 hours, was able to fill out status request form and show proof right through the National site. Took a few days... almost a week and then got matched to Executive Elite.

Had to send an email and fill out form.. took a few days but got matched after showing proof of status with Hertz. There's a link on Sixt Website, but it wasn't working for me.. so I sent an email to which they replied and gave me correct link for me to get status match completed.
